Consent Form for COVID-19 Testing in Secondary Schools

We are making arrangements for testing students prior to their return to school in September. The test will be carried out in school, dates and times will be confirmed shortly. We are asking for parents/carers to complete the Google form using this link https://forms.gle/jZ5ryrYBRm1FHZU39

This consent form is for participation in tests designed to detect asymptomatic coronavirus cases. Those taking the test will be supervised by trained staff. The lateral flow tests are quick and easy using a swab of your nose and throat. For under-18s, staff can oversee the swab process.
